ABSTRACT

Abstract Objective: To investigate the usefulness, reliability, quality, and content of Portuguese-language YouTube videos on COVID-19. Material and Methods: Three keywords selected on Google Trends were searched on YouTube, and the 60 first videos listed with each term were analyzed. Two calibrated researchers evaluated the reliability (DISCERN Modified Scale), the quality (Global Quality Score - GQS), and the usefulness of videos for the users (COVID-19 Specific Score - CSS). The number of views, likes, and engagement were also analyzed. The data were evaluated by the Mann-Whitney test and Spearman correlation (α=5%). Results: 59 videos were included. The average scores of quality, reliability, and usefulness were 3.0 (±1.1), 3.2 (± 0.8), and 1.5 (± 0.9), respectively. Two-thirds of the videos (64.4%) had low/moderate quality, and the majority (76.2%) were about signs and symptoms. The numbers of views (p=0.005), likes (p=0.006), and engagement (p=0.014) were significantly higher in moderate/good quality videos. The number of comments (p=0.007), duration of videos (p=0.004), and the DISCERN score (p<0.001) were significantly higher in videos made by health professionals. The general quality of the videos was positively correlated to the CSS scores, number of views, likes, and engagement (p<0.05). Conclusion: Most videos had moderate quality and reliability and low usefulness for the users.

ABSTRACT Introduction: The perception of prejudice against, and stigmatization of, sickle cell disease (SCD) leads the patient to perceive a different treatment, due to the disease stigma and maybe related to a worse quality of life (QoL). Objectives: Describe and evaluate the perception of the prejudice against the disease and its impact on the quality of life of patients with sickle cell disease. Methods: This is a cross-sectional study conducted between March 2019 and February 2020, with patients diagnosed with SCD. Patients were questioned about the perception of prejudice in any kind of situation, choosing between "Yes" or "No", not differentiating situations related to prejudice. To assess the QoL and impact of the disease, the volunteers answered a version of the SF-36 questionnaire translated and validated into Brazilian Portuguese. Results: In this study, 113 patients with SCD were followed up, 92% were classified as HbSS and the rest, divided between HbSC and HbS-β-0. Regarding the SF-36, the worst scores were in the summary of the physical components (mean 48.19 ± 21.51) and the physical aspect had the lowest mean (30.75 ± €42.65). When questioned if they had already perceived any kind of prejudice, including the SCD, 32.74% answered "Yes". For this comparison, there was a significant difference in the summary of the physical and mental components, with worse QoL for those who had already suffered prejudice. Conclusion: Patients diagnosed with SCD who reported perception of prejudice had statistically significant worse QoL, revealing the negative impact, that might lead to sadness and social isolation.

Abstract Background Brain edema is the leading cause of death in patients with malignant middle cerebral artery (MCA) infarction. Midline shift (MLS) has been used as a monohemispheric brain edema marker in several studies; however, it does not precisely measure brain edema. It is now possible to directly measure hemisphere brain volume. Knowledge about the time course of brain edema after malignant middle cerebral artery infarction may contribute to the condition's management. Objective Therefore, our goal was to evaluate the course of brain edema in patients with malignant MCA infarction treated with decompressive craniectomy (DC) using hemispheric volumetric measurements. Methods Patients were selected consecutively from a single tertiary hospital between 2013 and 2019. All patients were diagnosed with malignant middle cerebral artery infarction and underwent a decompressive craniectomy (DC) to treat the ischemic event. All computed tomography (CT) exams performed during the clinical care of these patients were analyzed, and the whole ischemic hemisphere volume was calculated for each CT scan. Results We analyzed 43 patients (197 CT exams). Patients' mean age at DC was 51.72 [range: 42-68] years. The mean time between the ischemic ictus and DC was 41.88 (range: 6-77) hours. The mean time between the ischemic event and the peak of hemisphere volume was 168.84 (95% confidence interval [142.08, 195.59]) hours. Conclusion In conclusion, the peak of cerebral edema in malignant MCA infarction after DC occurred on the 7th day (168.84 h) after stroke symptoms onset. Further studies evaluating therapies for brain edema even after DC should be investigated.


Resumo Antecedentes O edema cerebral é a principal causa de morte em pacientes com infarto maligno de artéria cerebral média. O desvio da linha média tem sido utilizado como marcador de edema cerebral mono-hemisférico em alguns estudos; porém, ele não mede de forma precisa o edema cerebral. Atualmente é possível mensurar diretamente o volume do hemisfério cerebral. O conhecimento sobre a evolução temporal do edema cerebral após infartos malignos da artéria cerebral média pode contribuir para o cuidado clínico desta condição. Objetivo Nosso objetivo é avaliar o edema hemisférico ao longo do tempo, em pacientes com infarto maligno da artéria cerebral média, tratados com craniectomia descompressiva. Métodos Os pacientes foram selecionados de forma consecutiva, em um hospital terciário, entre 2013 e 2019. Todos os pacientes apresentavam diagnóstico de infarto maligno de artéria cerebral média e foram submetidos a craniectomia descompressiva. Todas as tomografias computadorizadas de crânio destes pacientes foram analizadas, e o volume do hemisfério cerebral infartado foi mensurado. Resultados Analisamos 43 pacientes (197 tomografias de crânio). A idade média dos pacientes na craniectomia descompressiva foi 51,72 (42-68) anos. O tempo médio entre o ictus e a craniectomia descompressiva foi 41,88 (6-77) horas. O tempo médio entre o ictus e o pico do volume hemisférico foi 168,84 (142,08-195,59) horas. Conclusão O pico do volume cerebral em pacientes com infarto maligno de artéria cerebral média submetidos a craniectomia descompressiva ocorreu no 7o dia (168,84 horas) após o infarto. Mais estudos avaliando terapêuticas direcionadas ao edema cerebral seriam úteis neste contexto.

ABSTRACT Background: Sleep is a special physiological state that occurs cyclically. The probable role of sleep in our organic functions remains to be explored to clarify the impact of sleep on brain functions. Sleep deprivation is known to affect all parts of the brain separately and independently, but further research is needed on the impact of sleep disorders on attention, particularly the specific types of attention that are most affected, and whether there is such a correlation. Objective: To conduct a systematic review of the possible correlation between sleep disorders and attentional performance. Methods: A systematic review and search at PubMed, SciELO, and Cochrane scientific databases for articles published in the last 10 years was carried out using the following keywords: sleep, attention, and attentional performance. Inclusion criteria were the use of attention tests and sleep disorders. Of the 1398 articles found, 15 were selected and included in this review. Results: The number of publications evaluating sleep and sleep disorders has increased, but is still limited. Of all the functions normally assessed, patients with sleep disorders perform worse on attention tasks, especially with sustained attention. However, these data require further investigation due to the complexity and diversity of the disorders, the small sample size of the included studies, and the fact that few studies used standardized tests. Conclusions: Our findings indicate that the correlation between sleep and attention is strong but limited. Few studies are devoted exclusively to the extent to which sleep disorders interferes with attention.


RESUMO Antecedentes: O sono é um estado fisiológico especial que ocorre de maneira cíclica. Sabe-se que a privação do sono afeta todas as partes do cérebro de forma separada e independente, mas sobre o impacto que os distúrbios do sono podem ter sobre a atenção ainda são necessárias mais pesquisas, especialmente sobre os tipos específicos de atenção mais afetados e se existe tal correlação. Objetivo: Conduzir uma revisão sistemática da possível correlação entre os distúrbios do sono e o desempenho atencional. Métodos: Revisão sistemática e busca nas bases de dados científicos PubMed, Scientific Electronic Library Online (SciELO) e Cochrane por publicações dos últimos 10 anos foram realizadas usando as seguintes palavras-chave: sono, atenção e desempenho atencional. Os critérios de inclusão foram o uso de testes de atenção e a abordagem de distúrbios do sono. Dos 1.398 artigos encontrados, 15 foram selecionados e incluídos nesta revisão. Resultados: O número de publicações que avaliavam sono e distúrbios do sono cresceu, mas ainda são escassos. De todas as funções normalmente avaliadas, os pacientes com distúrbios do sono têm pior desempenho nas tarefas de atenção, especialmente com atenção sustentada. No entanto, esses dados requerem investigações adicionais em virtude da complexidade e da diversidade dos distúrbios, do pequeno tamanho das amostras e dos poucos estudos que utilizaram testes padronizados. Conclusões: Nossas conclusões mostram que a correlação entre sono e atenção é forte mas limitada. Poucos estudos dedicam-se exclusivamente a descobrir até que ponto os distúrbios do sono atrapalham a atenção.

Abstract: The Cerrado has been severely impacted by anthropogenic disturbances, with a tiny proportion of its original extent remaining in its southern portions. In the state of São Paulo, only 7% of this vegetation remains and relatively little is known about the biodiversity of these fragments. To fill this knowledge gap, we inventoried medium and large-sized terrestrial mammals of a neglected region, including a sustainable use protected area, Cajuru State Forest (CSF), adjacent native vegetation remnants protected by the Native Vegetation Protection Law and owned by a paper and cellulose company (Dois Córregos Farm; DCF), and their surroundings. We recorded 20 native mammal species, five of which are endangered with extinction, including the giant anteater, puma, and maned wolf. We found no significant differences in species richness between CSF and DCF but we found higher estimated species richness for the surrounding areas. Besides encompassing a larger and more heterogeneous area, the surrounding area still has a relatively high proportion (>30%) of native vegetation, providing habitat and resources for many species. The estimated mammal species richness for the entire study area was similar to that found in the largest protected area of the Cerrado in São Paulo State, the Jataí Ecological Station and its surroundings. We conclude that our study area still harbors a relatively rich community of large mammals and is important for the conservation of endangered species. This finding is particularly timing since the current State administration is considering to transfer the economic exploitation of CSF to the private sector. We therefore endorse an existing proposal to uplist this protected area, which still lacks a management plan, to a more restricted management category. Besides actions regarding this governmental PA, we argue that it is also important to involve the private sector in a conservation plan for the region.


Resumo: O Cerrado tem sido severamente impactado por distúrbios antrópicos e, especialmente na sua porção sul, poucos são os fragmentos remanescentes desse bioma. No estado de São Paulo, resta aproximadamente 7% da cobertura original de Cerrado e relativamente pouco se sabe sobre a biodiversidade desses remanescentes. Para preencher essa lacuna, inventariamos mamíferos terrestres de médio e grande porte de uma região pouco estudada que inclui uma Unidade de Conservação (UC) de uso sustentável (Floresta Estadual de Cajuru; CSF), áreas de vegetação nativa protegidos pela Lei de Proteção da Vegetação Nativa e pertencentes a uma empresa de papel e celulose (Fazenda Dois Córregos; DCF), e seus entornos. Registramos 20 espécies de mamíferos nativos, cinco dos quais ameaçados de extinção, incluindo o tamanduá-bandeira, a onça parda e o lobo-guará. Não encontramos diferenças significativas na riqueza de espécies entre CSF e DCF, mas encontramos maior riqueza estimada de espécies para o entorno. Além de abranger uma área maior e mais heterogênea, essa área ainda apresenta uma proporção relativamente grande (> 30%) de vegetação nativa, fornecendo habitat e recursos para muitas espécies. A estimativa da riqueza de espécies para toda a nossa área de estudo foi semelhante à encontrada na maior UC de Cerrado no Estado de São Paulo, a Estação Ecológica de Jataí e entorno. Concluímos que a área de estudo abriga uma comunidade rica de grandes mamíferos, sendo importante para a conservação de várias espécies ameaçadas de extinção. Essa descoberta é particularmente oportuna, uma vez que o governo estadual está considerando transferir a exploração econômica da CSF para o setor privado. Endossamos, desta forma, uma proposta existente para elevar esta UC, que ainda carece de um plano de manejo, para uma categoria de manejo mais restritiva. Além disso, argumentamos que também é importante envolver a iniciativa privada em um plano de conservação para a região.

ABSTRACT

Abstract This paper focuses on developing a reduced distance matrix to improve the computational performance during the protein interactions clustering. This proposed matrix considers as centroids two alpha carbon atoms from a protein structure and stores the distances between these centroids and the other atoms from this same structure. Each row in this matrix represents a database record and each column is a distance value. Through this build matrix, clusters were performed using K-Means Clustering. The precision and performance of this presented technique were compared with aCSM, RID and another distance matrix methodology that considers the distances between all atoms from each protein structure. The results were satisfactory. The reduced distance matrix obtained a high precision and the best computational performance.

ABSTRACT

O manejo de dispositivos cardíacos eletrônicos implantáveis de pacientes que evoluem a óbito tem sido motivo de controvérsia. Em nosso meio, não há recomendações uniformes, estando baseadas exclusivamente em protocolos institucionais e em costumes regionais. Quando o cadáver é submetido para cremação, além de outros cuidados, recomenda-se a retirada do dispositivo devido ao risco de explosão e dano do equipamento crematório. Principalmente no contexto da pandemia causada pelo SARS-Cov-2, a orientação e organização de unidades hospitalares e serviços funerários é imprescindível para minimizar o fluxo de pessoas em contato com fluidos corporais de indivíduos falecidos por COVID-19. Nesse sentido, a Sociedade Brasileira de Arritmias Cardíacas elaborou este documento com orientações práticas, tendo como base publicações internacionais e recomendação emitida pelo Conselho Federal de Medicina do Brasil.


The management of cardiac implantable electronic devices after death has become a source of controversy. There are no uniform recommendations for such management in Brazil; practices rely exclusively on institutional protocols and regional custom. When the cadaver is sent for cremation, it is recommended to remove the device due to the risk of explosion and damage to crematorium equipment, in addition to other precautions. Especially in the context of the SARS-CoV-2 pandemic, proper guidance and organization of hospital mortuary facilities and funeral services is essential to minimize the flow of people in contact with bodily fluids from individuals who have died with COVID-19. In this context, the Brazilian Society of Cardiac Arrhythmias has prepared this document with practical guidelines, based on international publications and a recommendation issued by the Brazilian Federal Medical Council.

ABSTRACT BACKGROUND AND OBJECTIVES: Knee osteoarthritis is a chronic disease that tends to affect elderly people and is characterized by severe pain, joint stiffness and limited function. In more advanced cases, the initial approach of knee osteoarthritis performed with traditional conservative pharmacological or non-pharmacological treatment may not present satisfactory results. There are alternatives for pain intervention with favorable results, with longer analgesia and that can help rehabilitation, such as analgesic peripheral nerve blocks, including the genicular nerve block, and radiofrequency ablation. The objective of this study is to report cases of genicular nerve block guided by ultrasonography, with favorable results in relation to analgesia and return of functional capacity. CASE REPORTS: Four elderly patients diagnosed with advanced knee osteoarthritis, with limited range of motion, and with severe chronic pain (mean visual numeric scale - VNS=7.75) were submitted to ultrasound-guided genicular nerve block, presenting significant pain improvement (mean VNS after 1 month of block=2.25) and regain of functional capacity. There were no cases of complications. CONCLUSION: Genicular nerve block guided by ultrasonography is a technique that can be performed as an intervention measure in pain. It presents satisfactory results of analgesia and regain of functional capacity, facilitating the rehabilitation process, and can be adopted in an outpatient clinic context.


RESUMO JUSTIFICATIVA E OBJETIVOS: A osteoartrite do joelho é uma doença crônica que tende a afetar pessoas idosas e é caracterizada por dor intensa, rigidez articular e limitação da mobilidade. Em casos mais avançados, a abordagem da osteoartrite do joelho com o tratamento conservador convencional farmacológico e não farmacológico pode não apresentar resultados satisfatórios. Nesse sentido, existem alternativas de intervenção em dor com resultados favoráveis, com maior tempo de analgesia e que auxiliam a reabilitação, como a realização de bloqueios analgésicos em nervos periféricos, como bloqueio dos nervos geniculares e a ablação por radiofrequência. Este estudo teve como objetivo relatar casos de bloqueios dos nervos geniculares guiados por ultrassonografia, com resultados favoráveis em relação à analgesia e retorno da capacidade funcional. RELATO DOS CASOS: Quatro pacientes idosos diagnosticados com osteoartrite de joelho em grau avançado, com limitação da amplitude de movimento e com dor crônica, com intensidade média de 7,75 pela escala visual numérica, foram submetidos ao bloqueio de nervos geniculares guiado por ultrassonografia, apresentando melhora importante da dor após um mês do bloqueio, com intensidade média de 2,25 e reganho da capacidade funcional, não havendo casos de complicações relacionadas aos bloqueios. CONCLUSÃO: O bloqueio dos nervos geniculares guiado por ultrassonografia é uma técnica que promoveu analgesia satisfatória e ganho da capacidade funcional, além de facilitar o processo de reabilitação, podendo ser realizada em caráter ambulatorial.

ABSTRACT BACKGROUND AND OBJECTIVES: Scalp dysesthesia is characterized by the presence of several localized or diffuse symptoms, such as burning, pain, pruritus or stinging sensations, without objective findings in the physical examination of the patient that can explain and link the existing symptomatology to some other etiology. The aim of this study was to describe a case of scalp dysesthesia, from its clinical and laboratory investigation and the conduct adopted. CASE REPORT: A 38-year-old male patient, first assigned to the Dermatology Service, with complaints of pruritus in the scalp for 5 years. In the consultation at the Pain Service, the patient complained of daily, intermittent and burning dysesthetic sensations, such as tingling and pruritus in the bipariethoccipital region, worsening with heat and associated with severe pain in the cervical region. Upon physical examination, evidence of excoriations associated with this pruritus was found. The patient received conservative pharmacological treatment, with significant improvement of the symptomatology after 3 months. CONCLUSION: Larger prospective studies are needed to further characterize the pathogenesis of scalp dysesthesia, to generate optimization of the available therapeutic options and consequently improve the care that is given to patients. This report corroborates with some findings already described in the literature, such as the association with cervical alterations and the improvement through the use of low-dose antidepressants and anticonvulsants such as gabapentin.


RESUMO JUSTIFICATIVA E OBJETIVOS: A disestesia do escalpo caracteriza-se pela presença de diversos sintomas localizados ou difusos, como queimação, dor, prurido ou sensações de picada, sem achados objetivos no exame fisico do paciente que possam explicar e ligar os sintomas existentes à alguma outra etiologia. O objetivo deste estudo foi descrever um caso de disestesia de escalpo, desde a sua investigação clínica e laboratorial, até a conduta adotada. RELATO DO CASO: Paciente do sexo masculino, 38 anos. Primeiramente foi ao serviço de Dermatologia com queixa de prurido em couro cabeludo há cinco anos. Na consulta do Serviço de Dor, o paciente queixava-se de sensações disestésicas como: formigamento e prurido em região biparieto-occipital que piora com o calor, associada à dor de forte intensidade, diária, intermitente e em queimação na região cervical No exame fisico, foram encontradas evidências de escoriações ligadas a esse prurido. O paciente recebeu tratamento farmacológico conservador, com melhora importante do sintoma após 3 meses. CONCLUSÃO: São necessários maiores estudos prospectivos para caracterizar ainda mais a patogênese da disestesia do escalpo, gerar otimização das opções terapêuticas disponíveis e, consequentemente, melhora na atenção prestada aos pacientes acometidos. Este relato corroborou alguns achados já descritos na literatura, como a associação com alterações cervicais e a melhora por meio do uso de antidepressivos em baixas doses e de anticonvulsivantes como a gabapentina.

ABSTRACT

The coronavirus disease (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), emerged in Wuhan city and was declared a pandemic in March 2020. Although the virus is not restricted to the lung parenchyma, the use of chest imaging in COVID-19 can be especially useful for patients with moderate to severe symptoms or comorbidities. This article aimed to demonstrate the chest imaging findings of COVID-19 on different modalities: chest radiography, computed tomography, and ultrasonography. In addition, it intended to review recommendations on imaging assessment of COVID-19 and to discuss the use of a structured chest computed tomography report. Chest radiography, despite being a low-cost and easily available method, has low sensitivity for screening patients. It can be useful in monitoring hospitalized patients, especially for the evaluation of complications such as pneumothorax and pleural effusion. Chest computed tomography, despite being highly sensitive, has a low specificity, and hence cannot replace the reference diagnostic test (reverse transcription polymerase chain reaction). To facilitate the confection and reduce the variability of radiological reports, some standardizations with structured reports have been proposed. Among the available classifications, it is possible to divide the radiological findings into typical, indeterminate, atypical, and negative findings. The structured report can also contain an estimate of the extent of lung involvement (e.g., more or less than 50% of the lung parenchyma). Pulmonary ultrasonography can also be an auxiliary method, especially for monitoring hospitalized patients in intensive care units, where transfer to a tomography scanner is difficult.

ABSTRACT

Resumo A obstrução venosa ilíaca ocorre em 20% a 30% da população. Nos portadores de insuficiência venosa crônica grave, essa prevalência é ainda maior, podendo chegar a 50% a 90% dos pacientes, situação em que essa obstrução é investigada pelo ultrassom intravascular. Métodos diagnósticos menos invasivos, como o Doppler vascular, ou mesmo invasivos, como a flebografia, podem falhar em seu diagnóstico. O tratamento endovascular dessas obstruções tem se demonstrado eficaz, seguro e associado a excelente resultado clínico e de perviedade, desde que princípios anatômicos e técnicos fundamentais sejam considerados e aplicados.


Abstract Iliac vein obstruction occurs in 20-30% of the general population. In patients with severe chronic venous insufficiency, this prevalence can be even higher, reaching 50-90% when the obstruction is investigated using intravascular ultrasound. Less invasive methods, such as venous Duplex Scanning, and even invasive ones such as venography may fail to diagnose the condition. Endovascular treatment of these obstructions is effective, safe, and associated with excellent clinical outcomes and stent patency rates, provided that fundamental anatomical and technical principles are considered and applied.

Abstract Objective: To perform a quantitative analysis of the brain volume of elderly individuals in a population-based sample. Materials and Methods: This was a radiological assessment and voxel-based quantitative analysis, with surface alignment, of 525 magnetic resonance imaging scans of individuals between 60 and 103 years of age who participated in the Saúde, Bem-estar e Envelhecimento (Health, Well-being, and Aging) study in the city of São Paulo, Brazil. Results: We noted a median rate of reduction in total brain volume of 2.4% per decade after 60 years of age. Gray and white matter both showed volume reductions with age. The total brain volume/intracranial brain volume ratio differed between males and females. Conclusion: We have corroborated the findings of studies conducted in the United States and Europe. The total brain volume/intracranial brain volume ratio is higher in men, representing a potential bias for the conventional radiological assessment of atrophy, which is typically based on the evaluation of the cerebrospinal fluid spaces.


Resumo Objetivo: Analisar, quantitativamente, o volume cerebral de idosos em uma amostra de base populacional em São Paulo. Materiais e Métodos: O estudo é uma avaliação radiológica e análise quantitativa baseada em voxel com alinhamento de superfície de 525 imagens de ressonância magnética de participantes de uma coorte de idosos (SABE - Saúde, Bem-estar e Envelhecimento) em São Paulo, Brasil, com idades de 60 a 103 anos, dos dois sexos. Resultados: Observamos redução média do volume cerebral total de 2,4% por década após os 60 anos de idade. A redução de volume ocorreu na substância cinzenta e na substância branca com a idade. A relação entre volume cerebral intracraniano e volume cerebral total diferiu entre homens e mulheres. Conclusão: Nós reproduzimos os achados de estudos prévios em populações americanas e europeias. A relação entre volume intracraniano e volume cerebral é maior em homens, o que pode representar fonte de viés na avaliação de atrofia radiológica convencional, já que essa avaliação é usualmente baseada em análise de espaços liquóricos intracranianos.

As fístulas aorto-cava são entidades raras e de etiologia variada, estando frequentemente associadas a significativa morbimortalidade. Acredita-se que o aumento da tensão da parede nos grandes aneurismas resulte em reação inflamatória e aderência à veia adjacente, culminando na erosão das camadas aderidas e na formação da fístula. O tratamento cirúrgico convencional tem altas taxas de mortalidade. Embolia pulmonar paradoxal e o vazamento são complicações temidas do tratamento endovascular. O uso de oclusor vascular associado a endoprótese bifurcada é boa opção no tratamento do aneurisma de aorta abdominal com fístula aorto-cava


Aortocaval fistulae are rare entities with a variety of etiologies and are very often associated with significant morbidity and mortality. It is believed that increased tension in the walls of large aneurysms can cause an inflammatory reaction resulting in adhesion to the adjacent vein and culminating in erosion of the adherent layers and fistula formation. Conventional surgical treatment has high mortality rates. Paradoxical pulmonary embolism and endoleaks are the most concerning complications linked with endovascular treatment. Using a vascular occluder in combination with a bifurcated endograft is a good option for the treatment of an abdominal aortic aneurysm with aortocaval fistula

OBJECTIVE: To evaluate the respiratory systems of male and female rats maintained in individually ventilated cages (IVCs) from birth until adulthood. METHODS: Female Wistar rats were housed in individually ventilated cages or conventional cages (CCs) and mated with male Wistar rats. After birth and weaning, the male offspring were separated from the females and kept in cages of the same type until 12 weeks of age. RESULTS: The level of food consumption was lower in male offspring (IVC=171.7±9; CC=193.1±20) than in female offspring (IVC=100.6±7; CC=123.4±0.4), whereas the water intake was higher in female offspring (IVC=149.8±11; CC=99.2±0) than in male offspring (IVC=302.5±25; CC=249.7±22) at 11 weeks of age when housed in IVCs. The cage temperature was higher in individually ventilated cages than in conventional cages for both male (IVCs=25.9±0.5; CCs=22.95±0.3) and female (IVCs=26.2±0.3; CCs=23.1±0.3) offspring. The respiratory resistance (IVC=68.8±2.8; CC=50.6±3.0) and elastance (IVC=42.0±3.9; CC=32.4±2.0) at 300 µm/kg were higher in the female offspring housed in ventilated cages. The ciliary beat values were lower in both the male (IVCs=13.4±0.2; CC=15±0.4) and female (IVC=13.5±0.4; CC=15.9±0.6) offspring housed in individually ventilated cages than in those housed in conventional cages. The total cell (IVC=117.5±9.7; CC=285.0±22.8), neutrophil (IVC=13.1±4.8; CC=75.6±4.1) and macrophage (IVC=95.2±11.8; CC=170.0±18.8) counts in the bronchoalveolar lavage fluid were lower in the female offspring housed in individually ventilated cages than in those housed in conventional cages. CONCLUSIONS: The environmental conditions that exist in individually ventilated cages should be considered when interpreting the results of studies involving laboratory animals. In this study, we observed gender dimorphism in both the water consumption and respiratory mechanics of rats kept in ventilated cages.

ABSTRACT Control strategies using insecticides are sometimes ineffective due to the resistance of the insect vectors.In this scenario new products must be proposed for the control of insect vectors.The complexes L-aspartate Cu (II) and L-glutamate-Cu (II) complexes were synthesized and characterized by elemental analysis, visible ultraviolet, infrared spectroscopy and potentiometric titration. The toxicity of these complexes was analyzed in Aedes aegypti (Diptera: Culicidae) larvae and Gram-negative and Gram-positive bacteria. The interaction between the ligands and the amino acid balance and the distribution of the species as a function of pH were discussed. The lethal concentration median (LC50) for Ae. aegypti larvae were: L-glutamic acid-Cu (II) - 53.401 mg L-1 and L-aspartate-Cu (II) - 108.647 mg L-1. The minimum inhibitory concentration (MIC) required for Staphylococcus aureus and Escherichia coli was: L-glutamate-Cu (II) 500-2000 mg L-1 and L-aspartate-Cu (II) 1000-2000 mg L-1. The concentrations demonstrated toxicity that evidence the potential of the complexes as bactericide and insecticide. Metal complexes formed by amino acids and transition metals are advantageous because of low environmental toxicity, biodegradability and low production cost.

Introduction Gangliogliomas are tumors commonly found in the temporal lobe and related to seizures; their appearance in the pineal region is rarely described. This report characterizes the first case of anaplastic ganglioglioma of the pineal region. Case Report The authors describe the case of a 32-year-old woman that developed progressive headache. An MRI investigation revealed a pineal tumor. The patient tested negative for biomarkers and underwent surgery through supracerebellar infratentorial approach and achieved gross total resection of the tumor in a challenging location. Pathological analysis revealed a biphasic neoplasm with the following two distinct phenotypes in separate fields: an immature neuronal component with several atypical mitoses and a mature astrocytic component with bipolar cells, microcysts, and eosinophilic bodies. The Ki67/MIB1 proliferation index was 20­30% in localized hotspots. Based on the pathological findings, the tumor was defined as an anaplastic ganglioglioma World Health Organization (WHO) grade III. Discussion/Conclusion Gangliogliomas are classified as glioneural neoplasms based on the histologic findings described as a mixture of neoplastic astrocytes and neurons. Moreover, these tumors represent 0.4­1.3% of tumors of the central nervous system. Authors describe de novo anaplastic ganglioglioma as 1% of the largest series. Gross total resection and adjuvant treatment may play important role in patients' prognostic. In this case, due to the malignant anaplastic component of her tumor, the patient received treatment with temozolamide and radiotherapy after gross total resection of the lesion.


Introdução Gangliogliomas são tumores comumente encontrados no lobo temporal e se relacionam com crises epilépticas; o aparecimento desses tumores na região da pineal é raramente descrito. Este relato caracteriza o primeiro caso de ganglioglioma anaplásico da região da pineal. Relato de Caso Paciente do sexo feminino, 32 anos, apresentou-se com cefaleia de piora progressiva. Investigação com ressonância magnética revelou tumor na região da pineal. Os biomarcadores para tumores da pineal foram negativos e a paciente foi submetida a microcirurgia com o acesso supracerebelar e infratentorial atingindo ressecção total da lesão. A análise patológica revelou neoplasia bifásica com dois fenótipos distintos em campos separados: um componente era composto por células neuronais imaturas com inúmeras mitoses atípicas e o segundo componente era composto por astrócitos maduros, microcistos e corpos eosinofílicos. Foi encontrado um índice proliferativo Ki67/M1B1 de 20­30%. Baseado nos achados anatomopatológicos, o tumor foi definido como ganglioglioma anaplásico grau III da OMS. Discussão/Conclusão Gangliogliomas são classificados como neoplasias glioneurais baseado nos achados histológicos descritos como misto de neoplasia neuronal e glial; esses tumores representam 0,4­1,3% de todos os tumores do sistema nervoso central. Ganglioglioma anaplásico de novo tem sido descrido em 1% nas maiores series de gangliogliomas. Ressecção total da lesão e tratamento adjuvante desempenham um papel importante no prognóstico dos pacientes. Devido ao componente anaplásico do tumor em questão, a paciente foi tratada com temozolamida e radioterapia após ressecção total da lesão.

Habitat loss and degradation is threatening mammals worldwide. Therefore, Protected Areas (PA) are of utmost importance to preserve biodiversity. Their effectiveness, however, depends on some management strategies such as buffer zones, which prevent/mitigate the impact of external threats and might increase the amount of available habitat for wildlife existing within reserves. Nevertheless, how intensively terrestrial mammals use buffer zones remains little studied, particularly in the Neotropical region. Aiming to analyse the use of a buffer zone (5 km wide) by medium and large-sized mammals, we modelled the occupancy probabilities of five species of conservation concern including local (interior and buffer zone) as a site covariate, simultaneously controlling for imperfect detection. Data collection was made with camera traps from April to September 2013 in a 9000 ha Cerrado PA ("interior") and in its surrounding area (39721.41 ha; "buffer zone"). This PA (Jataí Ecological Station) is immersed in a landscape where sugarcane plantations predominate in the northeastern of the state of São Paulo. We also conducted an inventory to compare the number and composition of species between interior and buffer zone. A total of 31 mammal species (26 natives) was recorded via camera traps and active search for sightings, vocalizations, tracks and signs. Occupancy estimates for Myrmecophaga tridactyla, Leopardus pardalis and Pecari tajacu were numerically higher in interior. On the other hand, Chrysocyon brachyurus had the highest occupancy in buffer zone, while the largest predator, Puma concolor, used both areas similarly. However, as the confidence intervals (95%) overlapped, the differences in occupancy probabilities between interior and buffer were weak for all these species. Additionally, regarding only the species recorded by cameras, the observed and estimated richness were similar between interior and buffer zone of the PA. Our data demonstrated that the buffer zone is indeed used by medium and large-sized mammals, including conservation-dependent ones. The lack of enforcement of current legislation regarding buffer zones is therefore a real threat for mammals, even when protection is guaranteed in the interior of protected areas.


A perda e degradação de habitat têm ameaçado os mamíferos no mundo todo. Dessa forma, a criação de Unidades de Conservação (UC) faz-se cada vez mais fundamental para sua preservação. Entretanto, estratégias de manejo são necessárias para que as UCs cumpram plenamente seus objetivos, tal como o estabelecimento de uma zona de amortecimento ao seu redor, a qual aumenta a área de habitat protegido e visa servir como barreira a impactos negativos externos. Contudo, o uso das zonas de amortecimento por mamíferos terrestres permanece pouco estudado, particularmente no Neotrópico. Com o objetivo de analisar o uso de uma zona de amortecimento (5 km de extensão) por mamíferos de médio e grande porte, nós modelamos a probabilidade de ocupação de cinco espécies de interesse conservacionista, incluindo local (interior e zona de amortecimento) como uma covariável de sítio, controlando, simultaneamente, as imperfeições na detecção. Os dados foram coletados por meio de armadilhamento fotográfico de abril a setembro de 2013 em uma UC de Cerrado de 9000 ha e em 39721,41 ha do seu entorno. A área de estudo (Estação Ecológica de Jataí) está imersa em uma paisagem de matriz canavieira no nordeste do estado de São Paulo. Nós também realizamos um inventário a fim de comparar a riqueza e a composição de espécies no interior e na zona de amortecimento. Foi registrado um total de 31 espécies (26 nativas), incluindo aquelas amostradas apenas por identificação de vestígios, de vocalizações e por observação direta. As estimativas de ocupação para Myrmecophaga tridactyla, Leopardus pardalis e Pecari tajacu foram numericamente maiores no interior, enquanto Chrysocyon brachyurus obteve a maior estimativa de ocupação na zona de amortecimento. Já o maior predador, Puma concolor, utilizou ambas as áreas de forma semelhante. Entretanto, como houve sobreposição entre os intervalos de confiança (95%), as diferenças nas probabilidades de ocupação entre interior e zona de amortecimento foram fracas para as cinco espécies analisadas. Além disso, considerando apenas as espécies registradas por armadilhamento fotográfico, a riqueza observada e a estimada foram similares no interior e na zona de amortecimento da UC. Nossos dados demonstraram que a zona de amortecimento é de fato utilizada por várias espécies de mamíferos de médio e grande porte, incluindo aquelas prioritárias para a conservação. Portanto, a falta de cumprimento da legislação vigente em relação ès zonas de amortecimento é uma ameaça real para as espécies de mamíferos, mesmo quando a proteção é garantida no interior das UCs.

ABSTRACT

Este estudo teve por objetivo descrever as estratégias cognitivo-comportamentais mais utilizadas por pacientes usuários de substâncias (n=52) e seus familiares (n=75) para lidar com as situações de risco durante o tratamento. A intervenção baseou-se na psicoterapia cognitivo-comportamental, e foram analisadas estratégias de fuga e enfrentamento. A análise de conteúdo destacou o suporte familiar para auxiliar o paciente no redirecionamento de novos hábitos envolvidos com o uso de drogas. A inclusão de estratégias direcionadas aos familiares dos dependentes foi importante para o treino de habilidades de mudança de comportamentos desadaptativos envolvidos no consumo de substâncias. Esses achados também ajudam na elaboração de futuras intervenções destinadas aos dependentes de drogas e seus familiares como forma de aumentar a efetividade da intervenção.


This study aimed to describe some cognitive behavioral strategies used by substance users (n=52), as well as their familiars (n=75), to handle with risky situation during the treatment. The intervention was based on the behavioral cognitive therapy, and we analyzed escape and coping strategies. The content analysis showed that the familiar support is important to help the users to change their habits linked with the substance use. The inclusion of some strategies directed to the familiars was important to change their social skills in order to handle with risk situations. These fidings also can support the development of new approaches to substance misuse users and their familiars in order to improve the quality of the interventions.
